Polypharmacy and multimorbidity are widespread concerns within our aging population, serving as significant drivers of medication-related harm that can lead to adverse health outcomes, preventable hospitalizations, and increased morbidity and mortality. Pharmacists can fulfill an essential role as knowledge brokers in delivering integrated, patient-centered polypharmacy stewardship. By implementing effective stewardship practices, polypharmacy stewardship can help mitigate medication-related harm in older adults facing inappropriate polypharmacy.

The course includes a short assessment, culminating in a certificate of completion from Positive Impact.
